How do I change the icon? After I replaced it and opened the mod folder it wasn't changed.

You did change it. The old icon is, as I stated, still present in memory (RAM).

I copied and pasted it and it showed! New problem now. The icon looks really blurry. I think this is because the clear version of the icon is 2:1041 and not 3:1041.

The reason is the icon's resolution (32x32). Try resizing it, Nearest Neighbor, to 64x64.

It is still blurry.
The original icon for cave story is 32x32.
I'm done for today.
I think the icon is fine for now, actually.

I just added in the boss fight with balrog's final form in my mod.
For it, I basically copied the original boss fight with curly there too.
I also used the same properties. But when I went to try it out, curly wasn't
there. When I changed npc tileset two to curly I could start the boss fight
but the balrog npc was messed up. I know curly has to be set to npc tileset 2.
How can I have both balrog and curly there at the same time?

I used note pad and it was jibberish.

that's because the data isn't written as plaintext
you won't see the 0 character when the value is supposed to be 0
use a hex editor like HxD, but BL supports opening npc.tbl, so I'm not sure why you want to open it unless for research

CE does too, if you use that I guess

Which Curly NPC were you using? NPC117 was used for her during the final Balrog battle.

um
npc.tbl has literally nothing to do with the issue you're having
my guess - check your TSC flags
it's probably a flag set when you talk to her in the hospital

I found out what the problem was!
Turns out curly was there, just invisible.
When I started the boss fight by talking
to the end of the boulder I found out that
curly was behind the boulder and that balrog goes
behind the boulder as well! How do I make it so
they appear in front of the boulder?

If you're using CE then you can delete and re-add the entities so the draw order is higher than the boulder, if you're using BL you can just set the draw order yourself (a higher number shows up in front of a lower number)
