My guess... only people who have seen one before will recognize the difference between a killer robot like Quote and a human.

The only people who realize Quote is a robot...

-Curly... [well... besides being a fellow well-made robot, she probably watches Quote bounce around armed to the teeth, shooting those bugs]

-Itoh [He jumps at his own shadow I bet... and he says "You aren't that savior are you?" Meaning it's debateable if he'd recognize Quote as a robot, if he hadn't been told that one was coming... [even if told it was to save them]]

-That guy that screams "KILLER ROBOT" [... probably programmed that way]

Everyone else knows because someone informed them about Quote. Even Booster didn't notice immediately.. he was like "who is your friend?" then... "!!!"

Also, every other robot looks completely different than Q and C...
Blue robots, Killer Robots... MALCO. Q and C are most likely just on a different level than the others, or a rarely seen type. (The other killer robot surely didn't look human.)

I'm guessing if not for the antenna that are kinda like some kind of rad headphones Q and C wear, it'd be hard even for someone like Booster to tell. (Persona 3's Aigis passed for a human at school...)

He does catch Sue and Quote with impeccable timing.... Now that I think about it, he's on an island full of danger; has the best hacking/computer skills of the lot and doesn't lose his cool to his sister; even though he's trapped in some room in the middle of nowhere... with no ... food....

He even has no trouble dealing with a newly hatched dragon...

You don't need to have eyes to fly a dragon, in fact animals can help out blind people (like dogs). And dragons are basically dogs in cave story.

Uhm, maybe it's because Kazuma originally first saw Quote through like, this teensy crack in the shelter. So he probably couldn't see most of Quote in the first place and since robots were pretty much extinct from the island he could possibly be a human.

Since he's already made an assumption, it's easier to believe Quote was human after the explosive was set. Also, Kazuma never saw Quote jump. It was stuck in shelter, door opens, cutscene, and then they drive off on that motor/whatever.
