No, Cave Story isn't a clone, Cave Story is an homage.
Underside is a Cave Story clone, which is a clean word for "cheap knockoff" or "ripoff". (The difference between an homage and a ripoff is that an homage is mostly original material, with some references thrown in, whereas a ripoff is mostly a copy, with only slight differences, and most differences boiling down to the story and gameplay not being as good.)

Yah, IMO that sounds more like game promotion (My game is like Cave Story, so people who liked it will like this too! etc.)

Which is the whole reason people make clones (or ripoffs) of games. So they can sell to fans of that game, without actually needing to come up with their own ideas.

They have stated they made their own engine. Some people believe this. Others don't. I'M GONNA USE SUE'S AND FIND OUT MWHAHAHAHAHA!

Edit: Sorry, I was downloading this while I typed the previous.... Maybe the file types required were editted? I also saw a block in the game that was made BY PIXEL!
*grumbles* Stupid boss keeps killing me...

 

It's quite possible and probably that the engine is original in it's coding - there's no obvious similarities in the EXE that my limited skills can see. They use OGGs for sound, map formats are different...

However, the incredible similarity gameplay wise still exists. They haven't done anything unforgivable, but they are too close to Cave Story to really be unique.
