The 1st major instance was when I nearly got one hit killed by a 524 damage critical hit from the Greater Storm Beast (I usually referenced it as Storm Beast X) in Star Wars: Knights of the Old Republic II. The 2nd time was in Final Fantasy 7, when I 1st triggered the Death Joker ([^v=]) from Cait Sith's 2nd tier Limit Break, Slots. Andwhyisit they would add an effect like that in (Especially since it only tends to occur when I don't cast the Death Force Enemy Skill to prevent Auto-Death effects...)?

Most other minor instances were sudden and/or offscreen attacks on multiple characters I was playing with.
(Geez... They say Leos are supposed to be brave. Guess I must have an emotional defect...)

Super Mario 64. I was quite a bit younger, but it still shocked and angered me.
I had FINALLY gotten 70 stars, shortened the endless stairway to the final Bowser fight, got ALL THE WAY THROUGH THE LEVEL to Bowser, and thrown him into a bomb. ALRIGHT. PRINCESS MAKEOUT TIME.
No. He just got back up again. I almost felt like cutting myself here, but I persevered. I threw him into another bomb (took a while), and the bastard STILL didn't die! He just STOMPED THE FLOATING ISLAND APART. Took me forever to kill him after that.
