During the Undead Core bossfight the Doctor (at least in the Nicalis translation) says he'll make Misery his puppet forever.
So like the title suggests.
Misery's appearance, her head, is meant to resemble a sock puppet.
The choice of the proportions and the way the mouth opens makes the resemblance pretty obvious but I think the joke got a bit obscured by her body staying relatively the same.
...You could make a similar connection with Sue as well, but it's a bit more open-ended what that could be.
I'm not even sure if this entirely counts as a theory but it's just something I noticed. I could also be entirely wrong and pixel just wanted to make her look scarier.
