OK, new idea (that one is coming to a dead end):
Maybe pooh black was the ghost of an assistant to the doctor/physician (not the bad guy)

 

Another thing about this boss is that it looks like a recycled sprite from the
Beta phase, as he looks almost like the Beta Balrog (Boss).

That's just my guess.

I wish there was. But I found these sprites in
the Cave Story Tribute site, in the beta information section,
I just ripped the sprites from the screenshots.

Hm... That's actually a really good idea. Let's see: These robots (or whatever they are, I'll go with robots) are designed to help their friends/owners/whatever. Balrog helps Misery, Pooh Black probably helped Dr. Gero. When he died, his ghost stayed at the place he was at most of the time when he was still alive. After Quote defeated him, he somehow came back to Dr. Gero, as seen in the cutscene. Or maybe he wasn't a ghost after all...

 

no, he looked pretty ghosty to me but he did help the gaudis (in the ending credits)...

No, the bright ones are real and hurt a lot (-5 is a lot, right?). The bad thing is that whether they hit you is random, and sometimes they just kept hitting me until death.
