I'd recommend getting clownacys dll mods and increase the screen size.
BUT! If you dont want to skip right by this! (Plz dont read if you want full screen!)
Quick guide to it:
(I will link this) There will be 2 files, the mods folder and the mod_loader_patcher.exe
Put both into the cave story folder, where DoConfig is.
Drag doukutsu to mod_loader_patcher
After that, go to the mods folder.
There will bequite a few things there, if you just want full screen, i would say delete these folders:
3ds_hud DELETE
debug_save DELETE
disable_image_protection DELETE
graphics_enhancement KEEP
ikachan_cursor DELETE
60fps KEEP
ogg_music DELETE
wasd_input DELETE
After that, go into graphics_enhancement.
There will be a settings.txt.
open it.
Heres where your choices divide:
You have 2 options: widescreen big or normal big. for Widescreen big copy this into the folder:
aspect_ratio_x 16
aspect_ratio_y 9
window_upscale 3
sprite_resolution 1
black_bars true For widescreen Slightly smaller, choose this:
aspect_ratio_x 16
aspect_ratio_y 9
window_upscale 2
sprite_resolution 1
black_bars true For Normal ratio big, choose this:
aspect_ratio_x 4
aspect_ratio_y 3
window_upscale 3
sprite_resolution 1
black_bars true For Normal Ratio slightly smaller, choose this:
aspect_ratio_x 16
aspect_ratio_y 9
window_upscale 2
sprite_resolution 1
black_bars true AFTERWARDS...
after you have done this,
delete the mod_loader_patcher.
go into the game. your settings should have switched! READ THIS TOO!
One more thing: set in DoConfig the window size as 640x480. trust me, the hacks for bigger screen will still work!

I'm pretty sure this guys wants to have actual fullscreen without blurry looks, not a large window.
I wonder though, would using a lower resolution (for the system itself, not just the game) make the game less blurry if used in fullscreen?
I still use DXGL for most mods myself. It can work with Steam overlay if added as a non-steam game. https://www.dxgl.info/
Results may vary, but for me it displays 98% of all Cave Story graphics properly with full screen only. (And doesn't have to change the resolution!) Once I installed and configured it, all I have to do now is copy the ddraw.dll it creates to each mod I'm using and set full screen.
(the Blade weapon in particular shows visual glitches at lvl 3, and occasionally a mod may display something odd; like on the Root, with the MC's head shows a square of black around it. Alpha issues?) If I knew what was so different about how that weapon displayed, maybe I could make a bug report.
I recommend only installing dxgl core files during setup. The ddraw com fix MAY have caused me problems with old RPGmaker games in the past. Not verified...
Also, you do have to have the requirements it specifies on the website for it to work.

After it's installed, open up Configure DXGL, set global options to video mode: aspect ratio stretch, and Scaling method: nearest neighbor (or play around with bilinear!). Everything else is probably fine. Then hit Add..., choose the game exe, and make sure Cave Story or the mod is set to full screen. If all goes well, it'll be full screen, and can be added to Steam as a non-steam game for overlay.
It does not work properly with clownacy's dll. Clownacy's dll is probably superior btw! But, this gives you more options.
Oh and you can always use OBS (Open Broadcaster Software) as a fake but proper full screen for almost any game. Works great for the occasional windowed only game. Might need specific hardware for it to run flawlessly?

for some reason and I don't know why it started working a week ago, but now it stopped working again
I tried changing all those things in compatibility but nothing works
it still is 640x480 in the top left corner and everything else is gray
640x480 windowed works normally, along with the super small one

EDIT: Just noticed something, not sure if it helps.
When I look really closely, at the bottom of the screen I can barely see my little bar at the bottom with all my icons.

All I can really say now is that Cave Story is one of those games that grow compatibility issues considering it was made on an old operating system, so if nothing else works, you may have to play the mod in windowed mode only.

Right-click on Doukutsu.exe, select properties, click on the "Compatibility" tab, tick "Run in 640x480 resolution", and hit OK. Do not have this set if running the game in windowed mode.
