This happened to me, as well, while I was playing a minimal run of The Wrath of Revorshkenahl (Around 6:09-6:12 in the video below). I'll admit, it is rather odd when this kind of thing happens, but it's not very common unless you know exactly how to make it happen every time.

What might fix this is if a <KEY command were added to the death event. Normally this makes you invincible. Or there might be a bug with Balrog's AI so that he always does damage to you, even if you have been <KEY'ed.
