Why don't I see a timer in hell?

I'm in hell for the first time, but I don't see any timer? Is it because I finish it for the first time? Or did I something wrong earlier in the game?

No, it doesn't.

However, the Counter won't show up if you've already locked yourself out of Sacred Ground (ie if Curly's dead).

After all, it's not like you can tell whether the Counter's in there or not without opening it, is it?
