I don't know how to use either of them, but I prefer Org because it's simpler and has a cooler name. Additionally, Org has lots of versions of OrgView with various pictures, whereas PxTone just has "pxtone Player".

Well, Cave Story doesn't really do anything special, it just uses really old APIs. This is also why the Japanese version has mojibake on non-Japanese Windows. It might be possible to improve performance by using a DirectDraw wrapper.
