Probably Aeon Genesis because that's how I first played the game, and like Random-storykeeper said, I think a lot of lines have more character than in the Nicalis version.
That's just my opinion tho ;)
Got the WiiWare demo, liked it, promptly forgot about it for 6-odd years, found it again last year and have adored it ever since.
It's a pretty flawless game.
If I didn't say Cave Story was one of the games I always come back to, I wouldn't be on this forum.

I wasn't really considering story when I thought that up. I just think it'd be a neat idea to have that sort of game within the CS universe.
Gaudis would make for some good enemies, and I totally agree with your playable character suggestions. Playing as Balrog and Misery would be badass.
How about a Cave Story beat-em-up like Final Fight or Streets of Rage? The weapons could be picked up and used strategically, and it'd be neat to showcase bosses in a new way.
Spur/Blade all the way. It's so satisfying to mow down a row of enemies once you get the Spur charged up all the way. The Knife's level 3 animation is super cool too, as well as it being the best weapon you'll arguably have until you get the Spur.
Super Missiles are good for boss killing, but...
I remember running across the Geobox demo vid like a year ago and thinking it was the coolest thing in the world.
A cool art style for Cave Story could be cel-shaded graphics, like Viewtiful Joe, Jet Set Radio or Okami. I think it'd look better in a 3D environment, but it would be really neat...
I'm not sure if taking Cave Story FULLY into the third dimension is a good idea. Just doesn't seem to work with the general formula/flow of the game. You'd have to change it up quite a bit for the experience to even be remotely fun. All things considered, try your best! It'll be interesting to...

Probably all the backtracking in Bushlands for the Jellyfish Juice. That coupled with all of the fetchquests you need to do to get the materials for the bomb just make it a chore, as well as having to get the Jellyfish Juice AGAIN if you want the Bubbler. It all amounts to just a big chore.
