It is the timer that activates when you enter Hell. It is also called the "Nikumaru Counter" in the AGTP translation.

Timing how long it takes for you to beat hell. You know those numbers that count upwards on the top-right of the screen when running Hell? That is the 290 Counter.

Of course it does. No 290 counter = no timer.
But I haaave it. I guess I'll see what it does when I can actually BEAT

It lets you record your time. Basically, it's for speed-runners who want to record their time. Hence the speech when you get the device.

To explain the translation error "Nikumaru Counter":
The 'nikumaru' part of the name in the untranslated version of the game reads 'ニクマル'. Like with Grasstown (kusamura,クサムラ, meaning 草村), phonetically written 'nikumaru' actually means "二九○" or 'two nine zero'. (As opposed to it being written 'two hundred and ninety')
二 : NI : two
九 : KU : nine
○ : MARU :"Circle (Sometimes Used For Zero)"
Don't really blame the AGTP for not figuring out what it really meant. It wasn't exactly obvious. Instead of translating it, they just romanized it.
It is likely a reference to an obscure timing chip or timer. As the game says, it has no buttons, so I'm not sure what the actual fuck activates this counter's real counterpart if it's not really just a timing chip.

Dude, it's the thing that gives you a time in the top right corner. It gets saved to the title screen when you win. If you get a good enough time the title screen's music changes and stuff.

By the way, it's a general rule of thumb to expect the average forums browser to have gotten the Best ending or at least know about it. Little use in using spoilers when mentioning Ballos, though it is kind to care about that kind of thing.
